Senior Foot Care

Registered Nurse Foot care performed in the comfort of your home, Service includes:

Full foot Assessment, circulation, neuropathy, and skin integrity evaluation. Personalized treatment care plan. Diabetes and Coumadin client education and support.

  • Toenail Trim
  • Thinning of thick toenails
  • Fungal nail management
  • Callus/Corn removal
  • Foot Massage
  • Hand / nail care

 

Medication Management 

Registered Nurse will review medication list, fill medication dispensers from recent list, Including weekly or monthly medication setup.

 

Assist with refills; calling MD for orders, Pharmacy contact for med reconciliation and MTM and pick up/ delivery of medications.

Assist with selecting and ordering pill dispensers, reminder devices and paper tracking.

Medication reminder calls.

Wellness Checks

 

Providing wellness checks, vitals and patient education on various medical conditions and caregiver training for enhanced support.

Review medical records, assist with scheduling, portal set up, telehealth visits with providers.

Nurse will provide gentle Range of motion and encourage chair exercise to Keep Moving:

  • Ankle Circles: Rotate your ankles slowly in both directions while sitting.
  • Toe Pulls: Try lifting just your big toe while keeping the others on the ground to build control.

  Check with your doctor to ensure safety for in-home gentle exercise.

RN Will encourage exercise as outlined by your physical or occupational therapist, as needed.

 

Professional Services

 

Complete assessments for assisted living, memory care, and adult day programs, including post-hospitalization evaluations.

Home safety evaluations, assistance with monitoring devices, and concierge services for appointments and medical results.
