RN Medication Management

 

Expert medication review, dispenser setup, refill assistance, and reconciliation to ensure safety and adherence.

Registered Nurse will review medication list, fill medication dispensers from recent list, Including weekly or monthly medication setup.

Assist with refills; calling MD for orders, Pharmacy contact for med reconciliation and MTM and pick up/ delivery of medications.

Assist with selecting and ordering pill dispensers, reminder devices and paper tracking.

Foot Care

Comprehensive foot assessments, personalized treatment plans, toenail trims, and management of common foot conditions.

Registered Nurse Foot care performed in the comfort of your home, Service includes:

Full foot Assessment, circulation, neuropathy, and skin integrity evaluation. Personalized treatment care plan. Diabetes and Coumadin client education and support.

Wellness Checks

& Education

Vitals monitoring, patient education, caregiver training, and assistance with medical record management.

Providing wellness checks, vitals and patient education on various medical conditions and caregiver training for enhanced support.

Review medical records, assist with scheduling, portal set up, telehealth visits with providers.

Transportation assistance (Uber/ Lyft) and stay to provide support during medical appointments and treatments including infusion therapy.

Medical records assist as requested (HIPPA compliant)

Comprehensive Professional Services

We provide essential professional services including detailed assessments for assisted living, memory care, and adult day programs.

We also offer home safety evaluations and assistance with setting up necessary monitoring devices.
